Crescent Parts & Equipment accepts applications from contractors or organizations with trained staff actively performing service, installation, or maintenance work in the HVACR industry. Please follow the link to our online credit application, as we no longer process paper forms. A list of required fields is below for your reference. The following list of company information must be submitted before consideration will be given in establishing either an open credit or COD account with Crescent Parts & Equipment Co., Inc.

Required fields will be noted with a red asterisk. Please note: You will not be able to submit your application unless all of the required fields are completed. We recommend having all required information ready before starting. You will have the ability to attach any supporting documentation at the end of the form. If you do not have data for a required field you may use NA/None or in the case of a phone number (111)111-1111, for example.

• Proof of registration with state and/or county where the business is located.

        a. A federal ID # or Social Security #
        b. State Charter # (if applicable)
        c. County or City Business License # (if applicable)

• EPA Certification # or a copy of the certificate (Certificate must be provided for Refrigerant purchases).

• Business Liability Insurance Provider and Policy number (Certificate of Insurance is not required).

• Commercial Trade References from other HVACR related businesses where the applicant currently receives "wholesale pricing".

• Bank reference information.

• For Sales Tax Exempt Customers - You may attach your exemption certificate if you have the form in an electronic format. If you do not, please fax your exemption certificate to 314-647-5891. The account will be made tax exempt upon receipt of the current form.

• All applications must be signed by authorized Owners/Officers.
