Credit Application and Conditions of Material Sales and Contract Services
Special Instruction(s):
This application should take about fifteen minutes to
complete, and you will need some or all of the following information:
- At least two (2) current trade
references with a contact name and email address for each.
- Contractor's License number.
- Please note that providing a personal guarantee will increase the likelihood of your
application receiving credit approval. If you (the applicant) are not the individual providing the
personal guarantee, you will need the name and email address of the individual providing that
guarantee.
- If sales will be tax exempt, you will be required to upload a copy of your tax
exemption certificate with this application. By state law, sales tax will be charged until an
exemption certificate is received and approved.
